Choice Connections was the perfect starting point for us when faced with the task of finding more appropriate care for our loved one. Lisa was with us through the whole process and has circled back numerous  times to make sure that our choice was still a good fit . “As an update, and a most sincere “Thank You,” Sara and I (with the help of Tender Care) moved Bob from the rehab to the Assisted Living.  By all accounts, the transition went smoothly and was relatively void of any agitation for Bob.  As we left him yesterday at his new Assisted Living home, he seemed to be resting comfortably and was awaiting a hearty lunch from Elisha and Dawn, both very welcoming and caring.

 It was our understanding that Hospice Services would come by later in the afternoon.  Of course, we will follow up to insure all is in place.
